At the customs line

The China health declaration on arrival: the form is gone, one duty remains

In short

Do I have to fill in a health declaration to enter China?

There is no longer a health declaration form to fill in. Since 1 November 2023, inbound and outbound travelers no longer file the Entry/Exit Health Declaration Form with customs. One duty survives: a traveler who has symptoms of an infectious disease, such as fever, cough, breathing difficulty, vomiting, diarrhoea or a rash, or who has been diagnosed with one, must voluntarily declare it and cooperate with health-quarantine checks.

Applies to

Everyone entering or exiting mainland China. The abolished form covered both inbound and outbound travelers.

Check before you rely on it

No opened source describes a currently required customs health app or 'fingertip' declaration service. The published rule is that the form is abolished.

Information checked:

This page exists to settle conflicting older advice. The form travellers still worry about was abolished on 1 November 2023. What is left is a duty for people who are actually unwell. Nothing here is medical or legal advice.

What changed

The form was abolished on 1 November 2023

From 00:00 on 1 November 2023, under the Frontier Health and Quarantine Law and its implementing rules, travellers entering or exiting China are no longer required to fill out the Entry/Exit Health Declaration Form. The mandatory requirement ceased for both inbound and outbound travellers.

What still applies

Symptomatic travellers must still declare

If you have symptoms such as fever, cough, breathing difficulty, vomiting, diarrhoea, a rash, or unexplained subcutaneous bleeding, or you have been diagnosed with an infectious disease, you must voluntarily declare your health status to customs and cooperate with temperature checks, epidemiological investigation, screening, and sample testing.

What to do

Five steps at the health-quarantine area

  1. 1.Expect no health declaration form. Since 1 November 2023 inbound and outbound travelers no longer file the Entry/Exit Health Declaration Form with customs.
  2. 2.If you have no symptoms of an infectious disease, walk through the customs health-quarantine area as normal; there is nothing to fill in.
  3. 3.If you have symptoms such as fever, cough, breathing difficulty, vomiting, diarrhoea, a rash, or unexplained subcutaneous bleeding, or you have been diagnosed with an infectious disease, declare it voluntarily to customs.
  4. 4.Cooperate with the health-quarantine measures customs applies: temperature checks, epidemiological investigation, medical screening, and sample collection and testing.
  5. 5.Understand the stakes before deciding to stay quiet: concealing or evading these measures carries legal consequences, and causing or seriously risking transmission of a quarantinable infectious disease can bring criminal liability.

The published facts

Every rule on this page, with the page it came from

One State Council report and one municipal portal citing the customs announcement. Sources read 2026-07-25.

The form was abolished on 1 November 2023

China's customs authorities announced the cessation of mandatory health declaration requirements for both inbound and outbound travelers. From 1 November 2023, inbound and outbound travelers no longer need to file a health declaration form with customs, according to a statement from the General Administration of Customs.

The legal basis for dropping it

On 31 October 2023 the General Administration of Customs of China issued an announcement that, under the Frontier Health and Quarantine Law of the People's Republic of China and its implementing rules, from 00:00 on 1 November 2023 travelers entering or exiting China are no longer required to fill out the Entry/Exit Health Declaration Form.

Symptomatic travelers must still declare

Individuals entering or exiting who exhibit symptoms of infectious diseases such as fever, cough, breathing difficulties, vomiting, diarrhoea, rash, or unexplained subcutaneous bleeding, or who have been diagnosed with an infectious disease, must voluntarily declare their health status to customs authorities.

And must cooperate with quarantine measures

A symptomatic traveler must cooperate with customs officials in health-quarantine procedures, including temperature checks, epidemiological investigations, medical screening, and sample collection and testing. The State Council report names temperature monitoring and epidemiological investigation among these measures.

Concealment has real consequences

Failure to disclose, or an attempt to evade quarantine measures, results in corresponding legal consequences. Those who cause the transmission of, or pose significant risks of transmitting, quarantinable infectious diseases face criminal liability. The residual duty is not a formality.

Why the page still exists after the form is gone

"You no longer fill a health declaration form" is itself the fact most travelers need, and it settles conflicting older advice. The State Council portal confirms the cessation for both inbound and outbound travelers, so the page reports the current rule rather than reconstructing a retired one.

In short

Health-declaration questions travellers actually ask

Starting with the one that settles it: do I have to fill in a form at all.

Do I have to fill in a health declaration form to enter China?

No. Since 1 November 2023 the General Administration of Customs stopped requiring the Entry/Exit Health Declaration Form for both inbound and outbound travelers. If you have no symptoms of an infectious disease there is nothing to fill in at the customs health-quarantine area.

Is there anything I still have to declare about my health?

Yes, if you are unwell. A traveler who shows symptoms of an infectious disease, such as fever, cough, breathing difficulty, vomiting, diarrhoea, rash, or unexplained subcutaneous bleeding, or who has been diagnosed with one, must voluntarily declare their health status to customs and cooperate with quarantine measures.

What counts as a symptom I need to report to customs?

The Beijing municipal portal, citing the General Administration of Customs, names fever, cough, breathing difficulties, vomiting, diarrhoea, rash, and unexplained subcutaneous bleeding, along with having been diagnosed with an infectious disease. Any of these triggers the voluntary self-declaration duty that survived the abolished form.

What happens if I hide that I am sick at the border?

Concealing your condition or trying to evade quarantine measures results in legal consequences. Someone who causes the transmission of, or poses a significant risk of transmitting, a quarantinable infectious disease can face criminal liability under the Frontier Health and Quarantine Law. The self-declaration duty is legally backed.

What quarantine steps might customs ask me to do?

If you self-declare or appear symptomatic, customs may apply temperature monitoring, an epidemiological investigation, medical screening, and sample collection and testing. The published sources describe these as the health-quarantine measures a symptomatic traveler must cooperate with on entry or exit.

What we could not confirm

Where the official sources stop, so the page does not send you to a form that no longer exists.

  • No opened source describes a separate customs app or 'fingertip' health-declaration service as currently required. The published rule is that the form is abolished, so this page does not send you to fill one.
  • No opened source lists which specific diseases count as quarantinable for the criminal-liability line; it names symptoms and the Frontier Health and Quarantine Law rather than a disease schedule.
  • No opened source states whether a self-declaration is made verbally, at a desk, or through a channel choice, so this page describes the duty rather than a form to complete.
  • Nothing here is medical or legal advice. If you are unwell on arrival, declaring honestly and cooperating is both the legal duty and the safer course.
